Which Questionnaire Should I Fill Out?If you have a problem relating to employment, you have these options:
- If there has been termination for reasons other than disability, or if there is more than one issue, the General Employment Questionnaire probably is a good place to start.
- If you feel you’ve been placed in a harassing or hostile work environment because of your sex/gender, race, color, national origin, age or similar characteristic, the Employment Harassment Questionnaire should be completed.
- The Disability Questionnaire is for someone who has not been hired, not been accommodated, or been disciplined or fired because of a disability or serious medical condition.
